Novalis stereotactic radiotherapy improved metastatic pituitary tumor.

چکیده
An 86-year-old woman presented with anterior and posterior pituitary deficiencies 5 months after chemoradiation for small-cell lung carcinoma. She had achieved remission, but gadolinium-enhanced T1-weighted magnetic resonance imaging revealed a pituitary mass with suprasellar extension, which was unrecognized 3 months previously, indicating metastasis (Picture 1A) (1). Because she did not want treatment other than hormone replacements, the tumor became enlarged over the subsequent 7 months (Picture 1B). Novalis (BrainLAB, Heimstetten, Germany) stereotactic radiotherapy (Picture 1C; total 39 Gy in 13 fractions) markedly reduced the tumor size and tumor marker levels without serious complications (Picture 1D; 5 months after irradiation). Her visual disturbance was significantly improved. Novalis achieves fractionated irradiation appropriate to tumor shape by intensity-modulated radiation therapy (IMRT) (2). The therapeutic effect of Novalis for pituitary metastasis, which is rare, has not been evaluated. If the tumor is large (>3 cm) and/or involves optic pathways, Novalis should be preferred over gamma knife radiosurgery.

Stereotactic Radiotherapy for Spinal Intradural Metastases Developing within or Adjacent to the Previous Irradiation Field – Report of Three Cases –
Results of stereotactic radiotherapy (SRT) for spinal intradural metastases developing inside or adjacent to the previous external-beam radiation therapy (EBRT) field are shown in 3 cases. One case of spinal intramedullary metastasis and two cases of intradural extramedullary metastases were treated using a Novalis shaped-beam SRT. Case 1 developed an intramedullary metastatic tumor in the C1 s...

BACKGROUND Benign tumors of the skull base are a challenge when delivering radiotherapy. An appropriate choice of radiation technique may significantly improve the patient's outcomes. Our study aimed to compare the dosimetric results of fractionated stereotactic radiotherapy between non-coplanar dynamic arcs and coplanar volumetric modulated arctherapy (Rapidarc®). METHODS Thirteen patients t...

متن کاملMetastatic pituitary tumor from renal cell carcinoma treated by fractionated stereotactic radiotherapy--case report.
A 63-year-old man presented with rapidly progressive visual field deficit and hypopituitarism including diabetes insipidus, 8 years after treatment for a renal cell carcinoma. Neuroimaging studies revealed a dumbbell-shaped pituitary mass that had destroyed the sellar floor and abutted against the optic apparatus. Fractionated stereotactic radiotherapy (SRT), employing computer-image integratio...
